Community Care Team - more

 

  1. Three Fellowship members coordinate the work of the team. Replacement Coordinators are identified as needed each spring as Fellowship Committees are formed. Ideally, retiring Coordinators will recruit their own replacements.
  2.  The Team Coordinators' role is to organize support activities to assure needs are met and to avoid confusion or duplication of effort.
  3.  The role of the Community Care Team is to ask the involved individual or family how we can be of assistance and then to provide the requested assistance within the limits of our resources.
  4.  Any Fellowship member who provides logistical or emotional support for another member through the Fellowship's coordinated efforts is a member of the Community Care Team.
  5.  Each fall all Fellowship members are surveyed to identify services they would be able to provide, with responses entered into a database by category of service. Any member may also volunteer by contacting the UFL office at 785-842-3339, or ufloffice@sunflower.com.
  6.  Coordinators solicit and coordinate specific services at the time a need is identified.
  7.  Protection of members's privacy is of the upmost importance. Permission must be gained directly from the involved member before personal information is shared with any Fellowship member.
  8.  Training for those making home and hospital visits is conducted by the Minister.  In addition, interested members may take part in "Mental Health First Aid Training" offered by Bert Nash Mental Health Center.
  9. Identification of community care needs occurs through a variety of formats:
    1. UFL website home page has contact links for e-mail to the Community Care Team and the Minister.
    2. Sunday bulletin has a brief, recurring notice of how to contact the Team.
    3. A notebook and/or individual cards are available on a special table in the sanctuary for those wishing to share a concern or ask for support.
    4. Any member may notify the Minister or a Team Coordinator of his/her own need or that of another member (with that member's permission).
    5. The Minister and Team Coordinators will identify a method for following up on significant concerns identified during Fellowship sharing of joys and concerns.
